In order to get a Phoenix Public Library Card you must:  

  1. Photo ID (employee ID, student ID, Driver's license, Passport) 
  2. Proof of address (Maricopa County Residents -- Free, all other Arizona residents $40/year)
    1. Utility bill
    2. Lease
    3. Mail (physical or electronic)  received within the last 30 days.
